This membership plan entitles you every year to 2 exams, 2 cleanings, 2 fluoride treatments, a set of cavity-detecting X-Rays, and an emergency exam. Additionally, you will also receive 20% off dental work. This includes items such as fillings, sealants, crowns, and extractions.
*Our plan is not insurance or a qualified plan under the Affordable Care Act. • Payment for treatment(s) not included in the plan is due at the time of service. • Plan discounts do not apply to the following procedures completed at our practice: •Orthodontics such as comprehensive orthodontic treatment and appliance therapy (CDT Code D8000-D8999) •Adjunctive General Services such as anesthesia (CDT Codes (D9000-D9999) and all procedures done by our oral surgeon• Our membership plan does not cover procedures completed by specialists outside our practice.
