AMPD Prime Details
This membership plan entitles you every year to 2 exams, 2 cleanings, 2 fluoride treatments, a set of cavity-detecting X-Rays, and an emergency exam. Additionally, you will also receive 20% off dental work. This includes items such as fillings, sealants, crowns, and extractions.
This membership only costs $40 a month per child 13 and under or $45 a month per adult over 14.
If you decide to pay in full, you will receive additional incentives such as $250-$500 off orthodontic treatment and up to 10% off sleep treatment. This offer cannot be combined with other discounts or promotions.
If you add up everything you are receiving with our plan versus paying the full price, you will be saving hundreds of dollars.

The plan has no limits and all procedures get a 20% discount. This does exclude orthodontics such as comprehensive orthodontic treatment and appliance therapy (CDT Code D8000-D8999). However, you can get a single-time flat discount. This also does not include Adjunctive General Services such as anesthesia (CDT Codes (D9000-D9999) and all procedures are done by our oral surgeon, Dr. Greg Bohle. Our membership plan does not cover procedures completed by specialists outside our practice.
